Job Summary:

We are seeking a skilled RN Care Manager to join our team at Boston Medical Center. This is a full-time position with an estimated annual salary of $80,000 - $110,000 based on experience.

About the Role:

The RN Care Manager will work closely with healthcare teams to promote quality and cost-effective outcomes for patients. This role involves assessing patient needs, identifying appropriate providers and facilities, and ensuring timely and cost-effective use of resources.

Key Responsibilities:

  • Assess patient needs and develop care plans in collaboration with healthcare teams
  • Identify and coordinate with appropriate providers and facilities throughout the continuum of services
  • Ensure timely and cost-effective use of available resources
  • Collaborate with healthcare teams to promote quality and cost-effective outcomes

Requirements:

To be successful in this role, you must have a strong nursing background, excellent communication and analytical skills, and the ability to work effectively in a fast-paced environment. A Bachelor's degree in Nursing (BSN) is preferred, but not required. You must also be licensed to practice as a Registered Nurse in Massachusetts and have 3-5 years of diversified clinical experience.

  • Experience in utilization management or case management
  • Knowledge of managed care concepts and health care delivery systems
  • Ability to assess patient needs and develop effective care plans
  • Strong interpersonal and verbal communication skills
  • Excellent analytical and data management skills
