Job Overview

Compensation and Benefits

Key Responsibilities

· The Clinical Case Coordinator is tasked with organizing and managing swing bed admissions, as well as reviewing emergency department admissions and discharges.

· Duties include administrative support, contributing to process enhancements, collaborating with referral specialists, and managing medical records.

· Operates under the general supervision of the Director of Nursing.

· Conducts financial assessments to determine patients' payment sources for inpatient stays, initiating the review process for timely payment.

· Responds to inquiries from medical offices, families, and external facilities.

· Provides case management, social services, utilization review, and discharge planning as scheduled.

· Identifies and coordinates swing admissions, ensuring all admission criteria are met.

· Reviews emergency department admissions/discharges and contacts patients for follow-up visits or transfers as necessary.

· Builds and maintains relationships with specialty offices to facilitate care coordination.

· Participates in interdisciplinary team meetings and utilization review discussions.

· Reviews patient charts upon request for counseling services.

· Assesses the need for interventions and community resources.

· Must be available for at least two Saturdays per month based on admissions.

Qualifications

  • 3-5 years of clinical nursing experience.
  • 1-2 years of experience in case management or a related field is preferred.
  • Current nursing licensure in California.
  • Bachelor's degree in nursing is highly preferred.
  • BLS certification required; ACLS certification preferred.
  • High School Diploma or GED is necessary.
  • Mandatory attendance for the first 90 days of employment.

Preferred Skills

  • Bilingual in Spanish is preferred.
  • Strong multitasking abilities.
  • Proficient in computer systems and software, including Microsoft Office, Athena, EMR, and Point Click Care.
  • Ability to thrive in a fast-paced environment.
  • Skilled in identifying and resolving patient care issues to enhance care quality.
  • Educates patients and families regarding health conditions and treatment options.
  • Effectively communicates changes in patient care plans to all relevant departments.
  • Knowledgeable about community resources and support services.
