Nurse Care Supervisor

We are seeking an experienced Registered Nurse to join our team as a Home Care Supervisor. In this role, you will be responsible for overseeing the care of chronically ill patients in their homes, ensuring the highest level of quality and patient satisfaction.

Key Responsibilities
* Supervise and coordinate the delivery of home care services to patients across the Keene and West Lebanon areas
* Provide clinical leadership and support to a team of registered nurses and other healthcare professionals
* Conduct regular assessments of patients' needs and develop personalized care plans to ensure optimal outcomes
* Collaborate with other healthcare providers and stakeholders to ensure seamless transitions of care
* Maintain accurate and up-to-date records of patient interactions and care activities
Requirements
* Current RN license in the state of New Hampshire
* At least 2 years of experience in a related field, preferably in nursing supervision or management
* Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
* Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments
* Reliable transportation and a valid driver's license
