Rolling Hills Healthcare Center, a member of the CommuniCare Family of Companies, is seeking an LPN MDS Nurse / Resident Assessment Coordinator (RAC) to join our team. As a key member of our Operations team, you will play a vital role in ensuring accurate and timely completion of mds assessments and coordination of the RAI process.

Competitive Compensation: We offer a salary range of $60,000 - $80,000 per year, depending on experience. In addition to competitive wages, we also provide Paid Time Off, Life insurance, LTD/STD, Medical, Dental, and Vision benefits, as well as a 401(k) Employer Match with Flexible Spending Accounts.

About the Role: The MDS Nurse RAC reports to the Executive Director and is responsible for ensuring that our centers are in compliance with federal and state regulations, as well as CommuniCare's guidelines and policies. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Requirements: To be successful in this role, you must have a valid LPN license in the state employed, 1 Year MDS Experience Required, and Three years of experience in a long term care environment preferred. Experience with the MDS/RAI process and/or case management is also preferred.
