Job Title: Occupational Therapist Home Health

Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in North Kansas City, Missouri.

About the Role:

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in their homes. As an Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patient's level of functioning and develop individualized plans of care to improve range of motion, muscle strength, and coordination.
  • Teach patients and their families on proper use of equipment, such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Communicate with patients' families and healthcare providers to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with agency protocol and Medicare/Federal guidelines.
Requirements:
  • Current license as an Occupational Therapist in the state of employment.
  • Minimum one-year experience as an OT in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands of the job.
