High Acuity Physical Therapy Specialist

We are seeking a skilled Physical Therapist to join our team at Amedisys, providing high acuity care in Hershey and surrounding areas.

About the Role:
  • As a Physical Therapist, you will be responsible for helping patients maintain, restore, and improve movement, activity, and function, enabling optimal performance and enhancing health, well-being, and quality of life.
  • You will work under a physician-approved plan of care, providing interventions, conducting re-examinations, modifying interventions as necessary, and performing discharge planning.
Key Responsibilities:
  • Conduct comprehensive patient examinations/evaluations at initial visits and develop patient-centered physical therapy plans of care in collaboration with physicians.
  • Ensure plans of care address all identified problems or document rationale if issues are not addressed.
  • Make necessary revisions to patient plans of care in conjunction with physicians and document continued care needs.
  • Provide physical therapy patient/client management requiring skills of a physical therapist, including interventions in accordance with physician-approved plans of care.
  • Supervise physical therapist assistants per state practice act and administrative laws.
  • Document each visit according to APTA documentation guidelines and accepted regulatory standards for inclusion in patient clinical records.
  • Provide resources for future therapy needs, promote wellness, and prevention.
  • Participate in in-services, chart audits, quality improvement activities (OBQI and OBQM), and peer-review activities.
  • Maintain current, unencumbered licenses to practice physical therapy specific to assigned states.
  • Possess graduate degrees from accredited physical therapy educational programs (CAPTE) and one year of related experience for PTs entering home healthcare to ensure knowledge and skills are sufficient for safely providing physical therapy services to patients.
