Job Description

As our Street Youth Outreach Worker-PT, you will play a vital role in supporting homeless unaccompanied youth aged 16-24. Your primary responsibility will be to engage with this demographic, establish rapport, and provide essential resources and referrals to ensure their well-being.

Key Responsibilities:
  • Conduct regular outreach services and develop meaningful relationships with homeless youth.
  • Visit various locations such as shelters, parks, schools, and libraries to identify and support youth in need.
  • Assess the needs of each individual and provide tailored referrals to community resources and SHL team members.
  • Collaborate with the SHL Case Manager or Counselor to provide extra support to youth with higher levels of need.
  • Maintain accurate records and documentation of interactions with participants using HMIS reporting mechanisms.
  • Promote effective communication within the Family Haven Alliance system of care and coordinate activities with MCMs.
Requirements:

To excel in this role, you should possess a Bachelor's degree in education or a related field, along with two years' experience working with at-risk youth and homeless populations. A background in mental health and addictions treatment is an added advantage.
