About the Role

We are seeking an experienced Registered Nurse (RN) to join our team as a Case Manager at Tenet Massachusetts, a leading healthcare provider in Massachusetts.

Job Responsibilities
  • Facilitate care along a continuum through effective resource coordination to help patients achieve optimal health and access to care.
  • Ensure that care is provided at the appropriate level of care based on medical necessity and assess patients for transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Integrate national standards for case management scope of services, including Utilization Management, Transition Management, and Care Coordination.
Requirements
  • Graduate of an accredited School of Nursing (BSN preferred)
  • Current RN licensure in State of Massachusetts
  • 2 years RN acute care hospital experience (acute care case management experience preferred)
