Job Summary

The Advanced Practice Provider (APP) within the Vascular ambulatory setting works collaboratively with a multidisciplinary healthcare team to provide comprehensive care to patients. This role requires a strong understanding of vascular health and the ability to work effectively in a fast-paced environment.

Key Responsibilities

  • Performs complete history and physicals, documenting in the medical record.
  • Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information, and diagnostic procedures where indicated.
  • Collaborates with the interdisciplinary healthcare team to make diagnoses of acute and complex chronic conditions.
  • Manages diagnostic tests through ordering, interpretation, performance, and supervision.
  • Formulates differential diagnoses by priority and diagnoses complications, ordering appropriate interventions.
  • Identifies expected outcomes from diagnosis and formulates and documents a plan of care to address complex acute and chronic healthcare needs.
  • Utilizes evidence-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
  • Implements and modifies the plan of care, prescribing diagnostic strategies and therapeutic interventions.
  • May perform advanced procedures consistent with privileges and competency validation.
  • Documents/dictates key components of patient progress via daily progress notes, transfer, and discharge summary, and/or clinic note where applicable.

Requirements

  • Master's Degree
  • National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant
  • BLS certification
