Job Overview

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Delta-T Group Inc. in Springfield, VA.

Role Summary

This is an exciting opportunity to make a meaningful difference in the lives of individuals with Intellectual and Developmental Disabilities (IDD). As a Care Coordinator, you will be responsible for providing everyday support and assistance to help them achieve their goals and live fulfilling lives.

Key Responsibilities:
  • Provide direct care and support to individuals with IDD, including assistance with daily living activities such as bathing, dressing, grooming, and toileting.
  • Assist with mobility and physical therapy exercises as needed, ensuring proper diet and nutrition, and assisting with feeding if required.
  • Follow individualized behavior support plans to address specific needs and challenges, using positive reinforcement and behavioral techniques to manage and improve behavior.
  • Assist with the development and maintenance of life skills, such as cooking, cleaning, and budgeting, and provide educational and recreational activities that promote personal growth and independence.
  • Maintain accurate and up-to-date records of individuals' progress, incidents, and any changes in their condition, advocating for their needs and preferences, and ensuring their rights and choices are respected.
Requirements:
  • Minimum of a High School Diploma or its equivalent.
  • Willingness and ability to provide direct care and supportive services as needed.
  • Valid Driver's License.
  • CPR/First Aid Certification.
Estimated Salary: $40,000 - $60,000 per yearBenefits:
