Job Summary

We are seeking a skilled and compassionate individual to join our team as a Home Health Care Professional - Person Centered Care Specialist. This is a full-time position working 7a-7p and 7p-7a shifts. The successful candidate will be responsible for assisting residents with activities of daily living, escorting them to scheduled programming, and participating in life enrichment activities.

Key Responsibilities

  • Assist residents with personal hygiene, bathing, dressing, transporting, and transfers.
  • Escort residents to scheduled programming, participate in outings and/or day trips, and encourage daily participation in scheduled programming.
  • Take and record vital signs and weights as ordered or required by regulations.
  • Participate in and attend all in-service training and education programs as scheduled.
  • Other duties as assigned.

Physical Demands and Work Environment

This position is very active, requiring frequent standing, walking, bending, kneeling, stooping, and climbing. The individual must use proper body mechanics to assist residents in their daily living and is required to lift or carry weight up to 50 lbs. and ability to push up to 250 pounds independently.

Salary and Benefits

The estimated salary for this position is $45,000 per year, based on national averages for Home Health Care Professionals in the United States. Benefits include health, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
