  • Job Summary
  • A Bachelor's prepared Social Worker assists with securing safe discharge placement of patients through helping patients/families identify discharge needs, verify health benefits, provide discharge planning for appropriate level of care post discharge including facilitating placement/transfer to another facility and securing necessary medications for discharge.
  • This role requires 6 months of related experience and a Bachelor's degree in social work or related field.
  • The ideal candidate will have excellent organizational, interpersonal, and communication skills and knowledge of community resources.
  • Essential Functions and Responsibilities include receiving referrals and assessing in a timely manner those patients and families/significant others who are referred to social work service, integrating social work plan into overall patient care through interdisciplinary collaboration, and providing social work services in the areas of comprehensive case management, discharge planning, and continuing care services.
  • Working Relationships include working directly with patients and/or customers, working with internal/external customers, and working with other healthcare professionals and staff.
