Speech Therapist Job Description

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Speech Therapist to join our team of skilled home healthcare providers. As a Speech Therapist, you will play a vital role in providing quality care to our patients in their homes.

Responsibilities:
  • Evaluate patients' level of functioning and hearing, and recommend mechanisms to enhance their hearing ability.
  • Provide initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish a Speech Therapy treatment plan in consultation with the physician and revise it with the physician's approval as necessary.
  • Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Speech Therapy.
  • Communicate with other team members and instruct them in speech pathology techniques that they may use while working with the patient.
  • Teach, supervise, and counsel the family and patient in the total Speech Therapy program and other related problems of the patient at home.
  • Record on a timely basis all evaluation data, treatments, and patients' response to therapeutic intervention.
  • Supervise the Home Health Aide every 14 days when Speech Therapy is the only professional discipline involved in the patient's care.
  • Record and report to the physician the patient's reaction to treatment and/or any changes in the patient's condition.
  • Participate in the development of periodic revision of the physician's plan of treatment, keeping him/her informed of the patient's status and obtaining additional orders if necessary.
Qualifications:
  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, and bending; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
