Salary

The estimated salary for this position is $65,000 - $80,000 per year, depending on location and experience.

Job Description

As a Hospice Transition Navigator at Enhabit, you will represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services.

You will integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. You will also serve as a public awareness representative and be responsible for education related to provided services.

Required Skills and Qualifications

To be successful in this role, you must:

  • Be a graduate of an approved school of nursing, therapy, or social work.
  • Be licensed in the state where you currently practice.
  • Have at least one year of experience in home care.
  • Have demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
  • Have basic demonstrated technology skills, including operation of a mobile device.
