MDS Coordinator

Optalis Health & Rehabilitation is seeking a skilled RN to serve as MDS Coordinator for the Care Center. In this role, you will ensure the organization's goals of positive outcomes in quality are achieved through effective processes, monitoring training, and implementation of systems that lead to accurate and timely MDS submissions. You will also oversee managed care residents, facilitate effective Center coordination through well-run IDT Meetings, and utilize the Optimus system. Ensure MDS compliance, resident-driven outcomes, and financial results that mirror the care given. Ensure the center remains in compliance with organization, local, state, and federal regulations.

Key Responsibilities:
  • Ensure accurate and timely MDS submissions
  • Oversight of managed care residents
  • Effective Center coordination through IDT Meetings
  • Utilization of the Optimus system
  • MDS compliance and resident-driven outcomes
Requirements:
  • Associate's or Bachelor's Degree in Nursing
  • Current Michigan License in good standing
  • Previous MDS experience required
  • Working knowledge of regulatory compliance in a Long-Term Care Setting
  • Previous experience with PointClickCare is a plus
