About the Role:

The ideal candidate will have experience in gerontological care, evidence-based medicine, and collaboration with healthcare professionals. Key responsibilities include performing comprehensive medical assessments, developing individualized care plans, and ensuring the highest level of care for patients.

Responsibilities:

  • Medical Assessments: Conduct thorough medical evaluations to identify and address the unique needs of each patient.
  • Care Planning: Develop and implement individualized care plans that prioritize patient comfort, dignity, and quality of life.
  • Collaboration: Work closely with healthcare professionals to ensure seamless communication and coordination of care.
  • Patient Education: Educate patients and their families on healthy lifestyle choices, disease management, and medication adherence.
  • Documentation: Maintain accurate and up-to-date medical records to facilitate effective care coordination.

Requirements:

  • Nurse Practitioner License: Current and active license as a Nurse Practitioner in the state of employment.
  • Certifications: National Board Certification as a Nurse Practitioner and DEA licensure.
  • Education: Master's degree in Nursing (MSN) from an accredited institution.
  • Experience: Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Skills: Excellent communication and interpersonal skills, ability to work independently, and proficiency in electronic medical record systems.
