Job Description

Cloverleaf Healthcare Center is hiring a skilled Physical Therapist to provide high-quality rehabilitation services in our state-of-the-art nursing home in Knightsville, Indiana. The ideal candidate will be responsible for developing and delivering personalized physical therapy programs to facilitate effective patient recovery.

Key Responsibilities:
  • To deliver exceptional physical therapy care within the PT scope of practice, adhering to established standards, department procedures, and productivity standards.
  • To participate in the evaluation process and help identify target lists, including short-term goals, long-term goals, and treatment plans.
Requirements:
  • Licensed Physical Therapist with BLS certification
  • Must be at least 18 years old
