Responsibilities:
- Conduct thorough assessments of patients' medical histories, current health status, and social environments to identify risks and develop targeted care plans.
- Collaborate with primary care physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination and effective communication.
- Develop and implement individualized care plans that address patients' unique needs and goals, with a focus on promoting independence, dignity, and well-being.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
- Participate in quality improvement initiatives to enhance the effectiveness and efficiency of our services.
- Travel to patients' homes, skilled facilities, and other community environments to deliver care and services in a variety of settings.
Qualifications:
- Master's degree in nursing or a related field, or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner, with a strong background in home health care or a related field.
- Excellent verbal and written communication skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to travel frequently, with a valid state driver's license and proof of adequate automobile insurance coverage.
Compensation:
As a Home Health Care Specialist with Matrix Medical Network, you'll enjoy a competitive compensation package, including a base salary of $105.00 or $110.00 per completed assessment, depending on the county. You'll also have the opportunity to participate in our mileage reimbursement plan and contribute to our 401(k) retirement plan.
