Responsibilities:

  • Conduct thorough assessments of patients' medical histories, current health status, and social environments to identify risks and develop targeted care plans.
  • Collaborate with primary care physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Develop and implement individualized care plans that address patients' unique needs and goals, with a focus on promoting independence, dignity, and well-being.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Participate in quality improvement initiatives to enhance the effectiveness and efficiency of our services.
  • Travel to patients' homes, skilled facilities, and other community environments to deliver care and services in a variety of settings.

Qualifications:

  • Master's degree in nursing or a related field, or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner, with a strong background in home health care or a related field.
  • Excellent verbal and written communication skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to travel frequently, with a valid state driver's license and proof of adequate automobile insurance coverage.

Compensation:

As a Home Health Care Specialist with Matrix Medical Network, you'll enjoy a competitive compensation package, including a base salary of $105.00 or $110.00 per completed assessment, depending on the county. You'll also have the opportunity to participate in our mileage reimbursement plan and contribute to our 401(k) retirement plan. Our Home Health Care Specialist will work in a variety of community environments, including patients' homes, skilled facilities, and other settings in Cheraw, South Carolina.
