Responsibilities
- Conduct thorough therapy evaluations to identify patient needs and develop effective treatment plans.
- Implement and monitor treatment plans, making adjustments as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in data collection and analysis to inform care decisions and improve patient outcomes.
Requirements
- Bachelor's Degree in Physical Therapy from an accredited institution.
- Current Washington Physical Therapist License (upon hire).
- National Provider BLS - American Heart Association (within 30 days of hire).
- National Provider Identifier (NPI) number (within 30 days of hire).
- 2 years of experience in physical therapy, with a focus on adult physical disabilities.
Preferred Qualifications
- 1 year of experience in home care or a related rehabilitation setting.
- Experience working with geriatric populations.
