LVN Position at Rosemead Healthcare Center

We are seeking a skilled Licensed Vocational Nurse to join our team at Rosemead Healthcare Center. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to a positive and supportive work environment.
Responsibilities- Implement the established plan of care for each assigned group of patients, working under the direct supervision of a registered nurse.
- Provide total care for chronically ill and technologically dependent patients, including administration of medications, performance of treatments, and provision of patient/family education as directed.
- Assist with the planning, coordination, and provision of individualized patient care in accordance with the established policies and procedures of the facility.
- Collaborate with all levels of nursing personnel, as well as with facility consultants, physicians, and department managers to ensure seamless patient care.
- Provide resident assessment-related education and participate in the identification of patient/family educational needs.
- Maintain knowledge of age-specific care, including identifying safety measures, physiological normal values, and assessing skin integrity, behavior, motor skills, and activities that place patients at risk.
- Communicate effectively with patients, families, physicians, additional healthcare providers, and co-workers in a clear, concise, and understandable manner.
- Current, valid, Licensed Vocational Nurse licensure to practice in the State of California.
- Current BLS certification.
- LVN graduate from an accredited School of Nursing.
- At least six months prior LVN experience in the past two years in a general acute care hospital setting or acquired equivalent competency appropriate to the type of patients that the facility provides care to.
- Ability to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language.
