Job Summary

The Clinical Excellence Team Lead is a dynamic and visionary leader who will spearhead our home health therapy services, ensuring seamless execution of clinical standards, staff development, and patient satisfaction. This role demands a deep understanding of home health therapy practices, clinical operations management, and staff supervision. You will work closely with our Clinical Operations Manager, Branch Director, and multidisciplinary care teams to drive excellence in patient care and outcomes.

Key Responsibilities

  1. Lead by example, setting the tone for a culture of excellence, empathy, and patient-centered care.
  2. Supervise and mentor therapists, providing constructive feedback and coaching to enhance their skills and confidence.
  3. Collaborate with the Clinical Operations Manager to develop, implement, and evaluate clinical policies, procedures, and best practices.
  4. Maintain a direct patient caseload, ensuring timely care and documentation that meets the highest standards of quality and patient satisfaction.
  5. Develop and lead training programs for staff, focusing on clinical skills, patient safety, and quality improvement.
  6. Participate in performance improvement activities, analyzing data to identify areas for growth and implementing targeted interventions.
  7. Assure compliance with regulatory requirements, agency policies, and professional standards.
  8. Contribute to the development of the branch's strategic plan, aligning with the organization's overall vision and goals.

Requirements

  • Bachelor's degree in a discipline-specific therapy program (Physical Therapy, Occupational Therapy, or Speech Language Pathology) or Master's degree in Speech Language Pathology.
  • Current state driver's license and auto insurance.
  • Active membership in a discipline-specific professional organization (APTA, AOTA, or ASHA).
  • Basic Life Support for Healthcare Provider (BLS HCP) certification from the American Heart Association.
  • OASIS certification within one year of hire date.
  • One year of experience as a licensed therapist in a clinical care setting.

Preferred Qualifications

  • Leadership experience in a home health or clinical setting.
  • Strong understanding of OASIS, ICD-10, and other relevant coding systems.
  • Experience with electronic medical records (EMRs) and other healthcare software.

Benefits
