Nurse Transfer Center Specialist

We are seeking a skilled Nurse Transfer Center Specialist to join our team at Bon Secours Mercy Health. This role will coordinate patient transfer logistics, ensuring seamless transitions between facilities and optimal patient care.
Key Responsibilities- Coordinate patient transfer logistics using established workflows
- Collaborate with clinicians, physicians, and hospital staff to ensure timely acceptance and access to appropriate care
- Utilize critical thinking and clinical decision-making skills to identify the best course of action for each patient
- Proficiently use EHR data entry, telephony systems, and office systems to support operations
- Monitor individual and team compliance and performance, implementing process changes as needed
- Associate Degree of Nursing or diploma (ADN) - Required
- 1 year of acute care experience - Required
- 3+ years of acute care experience - Preferred
