Job Description

We are seeking a highly skilled Hospice RN Case Manager to join our team at Scout Exchange. This is a fantastic opportunity to work in a dynamic and rewarding environment, providing exceptional care to patients in need.

About the Role
  • The successful candidate will be responsible for managing patient cases, ensuring timely and effective care delivery.
  • With a strong background in hospice care, you will develop and implement personalized care plans that meet the unique needs of each patient.
  • You will work closely with our multidisciplinary team to coordinate patient care, communicate effectively with families and caregivers, and provide emotional support during difficult times.
Requirements
  • Minimum 1 year of experience in hospice case management or end-of-life care
  • Strong knowledge of hospice care standards and regulations
  • Excellent communication, organizational, and time-management skills
  • Able to work independently with minimal supervision
