Job Title: Patient Care Coordinator

Oncology Specialists of Charlotte is a leading medical oncology practice dedicated to providing high-quality, compassionate care to patients with cancer and blood disorders.

We are seeking a highly skilled Patient Care Coordinator to join our team. As a key member of our support staff, you will play a vital role in ensuring the smooth operation of our practice.

Key Responsibilities:
  • Answer incoming calls in a prompt, courteous, and professional manner
  • Route calls and messages appropriately, following standard operating procedures (SOPs)
  • Provide personable, resourceful support to patients, visitors, and staff
  • Obtain pre-patient registration, demographic, and insurance information, and enter it into our Patient Management System (PM) or Electronic Health Record (EHR)
  • Request, collect, document, and assess all necessary records to ensure patient scheduling and continuity of care
  • Communicate with outside entities to obtain patient medical history records when necessary
  • Ensure all necessary medical information is obtained and entered into patients' electronic medical records ahead of appointments
  • Run and/or work daily reports to identify patients with unscheduled orders
  • Perform tasks related to Governmental, Insurance, and Quality Initiative Programs
Quality Initiative Programs:
  • Ensure communication is distributed to patients regarding program details
  • Generate and distribute Clinical Summaries from Electronic Health Record (EHR) to external providers
  • Communicate information regarding Patient Engagement Portal and assist with registration process
