Job Title: Advanced Practice Provider - Nurse Practitioner or Physician Assistant

LiveWell Choice, a subsidiary of Longevity Health, is seeking an Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team in Bucyrus, OH.

Job Summary:

We are looking for a highly skilled and compassionate Advanced Practice Provider to provide high-quality care to our members in long-term care facilities. The ideal candidate will have a strong background in geriatric care and be able to work autonomously in a fast-paced environment.

Key Responsibilities:

  • Provide collaborative primary care and services to a panel of Longevity Health members.
  • Execute and deliver Longevity Health Model of Care activities, treating in the most appropriate setting.
  • Develop and implement advanced care planning, embracing and executing treating in place model.
  • Facilitate and administer Health Plan clinical operations functions, including prior authorization, Part A and B services, Medication Reviews, HEDIS and STARs quality gap closure.
  • Serve as a trusted advisor to members, loved ones, and nursing home staff, educating care teams on member needs and early identification of changes in condition.
  • Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members on members' care needs, goals of care, treatment plans/changes.

Requirements:

  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required.
  • Will allow acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physician's Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
  • DEA License required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Valid state driver's license with a good driving record and proof of automobile insurance required.

Education and Training:

  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master's Degree from an nationally accredited program.

Knowledge and Experience:

  • Minimum of 6 months experience as a Nurse Practitioner/Physician's Assistant AND has some experience with geriatric/hospice or palliative care population prior to becoming an NP/PA OR is a licensed and board-certified new graduate NP/PA supervised by an experienced NP/PA in the LH New Graduate Fellowship program for 6 months and successfully completes all competencies for independent practice as an LH APP.
  • Experience working in the nursing home environment is strongly desired.
  • Computer/Technology proficiency required.
  • The ability to engage and communicate effectively with physicians, patients, family members - excellent people skills.
  • Self-directed but functions well in a team environment.
  • Excellent clinical skills - assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
  • Can function independently, demonstrated critical thinking skills, organization, and prioritization skills.
