About the Role:
* Conduct thorough in-home assessments, including medical history reviews, system evaluations, medication reconciliations, vital sign checks, and comprehensive physical exams
* Perform evidence-based practice screenings, such as point-of-care testing (as needed)
* Identify potential diagnoses for active medical management and inform member's primary care physician of any gaps in care
* Communicate findings through clear, concise reports to inform member's PCP
* Recognize urgent or emergent situations and intervene appropriately
* Educate members on disease processes, medications, and compliance

Requirements:
* Active NP license or ability to obtain by start date; licensure must be unencumbered with no open disciplinary actions
* Current national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care; or ability to obtain within 6 months
* Ability to complete job requirements, including lifting a 30-pound bag and navigating varied dwelling conditions
* Reliable transportation for home visit assessments
Preferred Qualifications:
* 1+ years of clinical experience in highest level of education; clinical setting
* May be requested to obtain additional licensure in other areas
* Experience in gerontology, cardiology, internal medicine, or endocrinology
* Home Health care or home visit experience
* Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages may be beneficial

Please note that employees must remain in the role for at least 12 months before being eligible for student loan repayments. Employees must also stay in an Advanced Practice Clinician or Physician role within UnitedHealth Group Inc. for 36 months to receive the full benefit of the student loan repayments.
