Job Summary

We are seeking a highly skilled and compassionate Mental Health Worker to join our team at WES Health System. As a key member of our team, you will provide intensive care and support to children, adolescents, and their families in their homes, schools, and communities.

Key Responsibilities

  • Assist therapists in providing individual, family, and group therapy sessions using evidence-based treatment modalities.
  • Empower clients and families to identify and build upon their strengths.
  • Maintain regular collaboration with all systems involved with clients and families.
  • Provide emotional support, structure activities, and positive behavioral reinforcement.
  • Assist in reducing the likelihood of hospitalization and residential treatment.
  • Evaluate client and family needs, interests, and response to treatment.
  • Connect families with community resources and supports.
  • Follow up on clients requiring hospitalization.
  • Intervene as needed to address problem behaviors.
  • Execute discharge plans and make referrals to community resources.
  • Schedule and co-facilitate interagency and discharge meetings.
  • Obtain and update consents.
  • Make recommendations and referrals to other programs.
  • Complete and submit progress notes and documentation in a timely manner.
  • Be flexible in scheduling appointments to meet client needs.
  • Contribute to quality improvement and outcome measurement tracking.
  • Meet productivity standards and attend mandatory trainings.
  • Participate in supervision, family meetings, and staff meetings.

Requirements

  • Bachelor's degree in a Human Services discipline, such as Psychology, Counseling, or Clinical Social Work.
  • One year of verified experience in case management or counseling with children and families.
  • Valid FBI clearance, child abuse history clearance, and driver's license in good standing.
  • Participation in CBH mandatory in-home services.
