LPN - Clinic II (Meds) - Healthcare Professional

As a Licensed Practical Nurse (LPN) in our Clinic II (Meds) team, you will provide high-quality patient care in an outpatient setting under the direction of a Registered Nurse and medical orders from a Practitioner. Your primary responsibilities will include:
Key Responsibilities:- Medical Record Management: Review and prepare medical records for assigned patients, initiate follow-up for medical records as requested, and review patient needs to identify and prepare orders for authorization by the Practitioner.
- Nursing Procedures: Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by the Practitioner, and prepare medications for Practitioners to use.
- Point of Care Testing: Perform point of care testing as authorized, provide laboratory, x-ray, and other reports to the Practitioner for review, and notify patients of normal results as directed by the Practitioner.
- Documentation and Communication: Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards, and observe patients/family responses to the plan of care.
- Team Collaboration: Collaborate with RN/Practitioner to revise care plans when needed, demonstrate and apply competency in basic nursing skills and use of equipment, and maintain current knowledge and skills needed to perform the position.
- Emergency Response: Perform efficiently in emergency patient care/clinic situations, follow established procedures, and inform appropriate Practitioner/RN.
- Supply Management: Maintain, order, organize, rotate, and stock supplies, and schedule patients for routine care, acute care, and addressing care gaps.
- Additional Responsibilities: Participate in orientation of new personnel, demonstrate sufficient knowledge, skills, and abilities in the area of specialization, and perform clean/sterile technique, set up for procedures, and assist with clinical procedures within department scope.
- Education: Hawaii State Board of Nursing approved curriculum for LPN licensure.
- License and Certification: Licensed Practical Nurse (Hawaii) required at hire, Basic Life Support from American Heart Association.
- Skills and Abilities: Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
- Previous Nursing Experience: Previous nursing experience, preferably in a clinic setting.
- Technical Skills: Demonstrated knowledge of and skill in word processing and spreadsheet PC applications.
