Aveanna Healthcare is a leading provider of home healthcare services, dedicated to delivering high-quality care to patients in the comfort of their own homes. We are seeking a skilled Physical Therapist (PT) to join our dynamic team in LeHigh Acres and surrounding areas.

Job Summary

We are looking for a compassionate and experienced Physical Therapist to provide physical therapy services to patients in need of rehabilitation and recovery. As a PT with Aveanna, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.

About the Role

This is a PRN position, offering flexibility and autonomy to manage your schedule and caseload. You will have the opportunity to work with patients of all ages and backgrounds, providing assessment, evaluation, and treatment to help them achieve optimal mobility and function.

Responsibilities:
  • Perform initial and ongoing assessments to evaluate patient mobility, coordination, strength, and current level of function.
  • Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
  • Develop and implement individualized plans of care, defining physical therapy needs, goals, progress, and expected outcomes.
  • Deliver physical therapy services to comply with prescribed physician orders.
  • Maintain accurate and timely documentation of patient progress, modifying goals and treatment as needed.
Requirements

To be considered for this role, you must possess:

  • A degree in Physical Therapy from an accredited university/program.
  • A current, unrestricted state license as a Physical Therapist in the state of practice.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.
