Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at D-H Lebanon-MHMH. This is a rewarding opportunity to provide high-quality patient care in an inpatient/procedural setting.

About the Role

The successful candidate will be responsible for delivering safe and effective nursing care to patients under the supervision of a Registered Nurse. Key responsibilities include:

  • Conducting thorough patient assessments, documenting findings, and developing individualized care plans.
  • Administering medications as prescribed, including intravenous medications after completing required training.
  • Providing assistance with activities of daily living, nutrition, and hydration, and monitoring patients' progress.
  • Reporting any changes or concerns to the interdisciplinary team and Registered Nurse.
  • Maintaining accurate records, adhering to policies, and ensuring a safe environment of care.
Qualifications

To be considered for this position, candidates must possess:

  • A graduate degree from an accredited school of practical nursing.
  • An active LPN license in the State of NH.
  • Basic Life Support (BLS) certification for Healthcare Providers.
Compensation and Benefits
