About the Role

We are seeking a dedicated and compassionate Patient Care Advocate to join our team at Intermountain Health. As a vital member of our healthcare community, you will play a critical role in providing exceptional care to patients and their families.

Job Summary

The Patient Care Advocate is responsible for assessing patient needs, developing and implementing care plans, and coordinating services to ensure seamless transitions and optimal outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with interdisciplinary teams.

Salary and Benefits

The hourly rate for this position is between $30.94 and $47.77, depending on experience. In addition to competitive compensation, we offer a comprehensive benefits package that includes medical, dental, and vision coverage, as well as retirement savings options and paid time off.

Key Responsibilities

  • Assessing Patient Needs

Conduct thorough assessments of patient medical history, mental health, and current presentation to understand their unique needs and develop personalized care plans.

Care Planning and Coordination

Work collaboratively with patients, families, and healthcare teams to develop and implement effective care plans, ensuring timely discharges and optimal outcomes.

Service Coordination

Identify and connect patients with necessary community resources, treatment programs, and support groups to enhance their overall well-being.

Requirements and Qualifications

  • Master's Degree in Social Work

A Master's degree in Social Work from an accredited institution is required, and a valid state license as a Clinical Social Worker (CSW) or Licensed Clinical Social Worker (LCSW) is also necessary.

Excellent Communication Skills

Exceptional written and verbal communication skills are essential for building strong relationships with patients, families, and healthcare teams.

Preferred Qualifications

  • Experience in Patient Care

Prior experience in patient care or social work is highly desirable, as it demonstrates a deep understanding of the complexities involved in delivering high-quality patient care.
