About the Role:

The Access Center RN plays a vital role in the management of patients pending admission to a gilchrist program. This position ensures timely resolution of delayed admissions, facilitating access to needed care for patients and referrers alike. The Access Center RN maintains a database tracking delays, providing valuable insights for workflow improvements and program development opportunities.

Responsibilities:

  • Manage patient pending admission to gilchrist programs
  • Follow up on delayed admissions and facilitate timely resolution
  • Maintain database tracking delays and suggest workflow improvements
  • Participate in Leadership Team and assist in Field Access RN and coordinator development

Requirements:

  • Diploma, ADN, or BSN from an accredited institution
  • Two to four years of nursing experience, with hospice admission experience preferred
  • Strong time management, interpersonal, and communication skills
