Job Summary

The Patient Care Navigator is a critical role at Rush University Medical Center, responsible for assisting nurses and social workers in providing outreach efforts to patients in need. This position requires a strong understanding of ambulatory settings and the ability to communicate effectively with patients, families, and healthcare teams.

Key Responsibilities
  • Monitor patient lists and provide outreach for follow-up appointments and new patient assessments.
  • Prepare and deliver medical records and correspondence to referring agencies and primary care providers.
  • Document all correspondence in electronic medical records and participate in meetings as assigned.
  • Conduct Health Risk Assessments and provide assistance with insurance enrollment, verification, and redetermination.
  • Coordinate transportation for patients as needed and perform other duties as assigned.
Requirements
  • High School diploma or GED and 1 year of experience in Electronic Medical Records.
  • Knowledge of ambulatory settings and basic Microsoft Office skills.
  • Excellent written, verbal, and interpersonal communication skills.
  • Ability to multi-task, function independently, and be self-motivated.
Preferred Qualifications
  • Associate's degree in a health-related field, such as social work, psychology, or medical terminology.
  • Bilingual skills.
