Job Description

We are seeking a skilled Clinical Healthcare Specialist - Physical Therapy Assistant to join our team at Providence Home and Community Care. As a Clinical Healthcare Specialist, you will play a vital role in providing high-quality patient care in home health settings.

Key Responsibilities:

  • Work under the supervision of a physical therapist to provide skilled physical therapy services to patients with various diagnoses and disabilities.
  • Tailor treatment plans to meet individual patients' unique needs and abilities.
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.

Requirements:

  • Associate's Degree in Physical Therapy from an accredited program.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • Oregon Physical Therapy Assistant License upon hire.
  • Washington Physical Therapist Assistant License (if practicing in Washington state).
