Responsibilities:
We are seeking a skilled Physical Therapist to join our team, responsible for:
- Collecting and recording patient care data that follows format and guidelines of the patient care service, regulatory agencies, and third-party payers
- Initial evaluation, daily notes, reassessments/progress reports, discharge plans, and communication with members of the team
- Daily billing and following department guidelines for productivity
- Preparing for evaluations in accordance with department policies/procedures and standards of practice
- Selecting and administering tests and measures
- Synthesizing evaluation results to formulate the diagnosis, goals, prognosis, and plan of care
- Establishing treatment plans based on evaluation results, goals, and best practice
- Collaborating with interdisciplinary teams, patients, and patient support systems
- Identifying the need for modifications to the plan of care and goals
- Initiating and completing discharge plans
- Providing education/training to patients and/or patient support systems
- Functional training, including ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription and training, and community reintegration
- Responsible for treatment, plan of care, exercise prescription for patient caseload
- Providing interventions to patients that are safe, effective, and in compliance with JAG
- Overseeing support staff of PTA, ATC, PT aides
- Attending annual JAG Billing and Coding seminar
- Promoting JAG in the community through lectures/presentations
Required Skills and Qualifications:
To be successful in this role, you should possess:
- Bachelor's or Master's Degree required, Doctorate degree preferred
- Licensed Physical Therapist in the state practicing in
- Completed state-regulated CEU requirement in the appropriate state (NY, NJ)
- Valid CPR License
- Experience/knowledge with an EMR software is preferred
- Excellent oral, written communication
- Strong clinical decision-making skills
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
