**Responsibilities:**

  • Develop and implement individualized plans of care for patients requiring intensive rehabilitation services.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care and achievement of patient goals.
  • Assess patients' needs and develop strategies to promote independence and improve functional outcomes.
  • Provide education and support to patients and their families on disease management, self-care skills, and resource utilization.
  • Maintain accurate and up-to-date documentation of patient care and progress.

**Requirements:**

  • Bachelor's degree in Nursing (BSN) or Associate's degree in Nursing (ASN)
  • Illinois state RN License in good standing
  • BLS (American Heart Association) certification

**Compensation & Benefits:**

We offer a competitive salary of $85,000 - $110,000 per year, commensurate with experience, plus a comprehensive benefits package including medical, dental, and vision insurance, 401(k) matching, and paid time off.

**Estimated Salary:**$95,500
