UPMC Home Health is hiring a full-time LPN for the Uptown, Shadyside, Squirrel Hill, East Liberty, Homewood and surrounding areas. With a competitive salary of $ $65,000 to $85,000 per year, this is an excellent opportunity to join our team as a Licensed Practical Nurse.
About the JobWe are seeking a skilled and compassionate Licensed Practical Nurse to provide high-quality patient care in a dynamic and supportive environment. As a member of our healthcare team, you will be responsible for delivering personalized care to patients in their homes, promoting their independence, and improving their overall health outcomes.
Key Responsibilities:- Deliver exceptional patient care, prioritizing their needs and preferences.
- Assess patient conditions, develop and implement individualized care plans, and collaborate with interdisciplinary teams to achieve optimal results.
- Administer medications, perform various medical procedures, and maintain accurate records.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Promote patient education, empowering them to manage their health and make informed decisions.
To succeed in this role, you must have:
- A valid LPN license in the state where you practice or a compact state.
- 1+ years of experience in healthcare, preferably as an LPN.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Basic computer skills and proficiency in electronic health records.
- Demonstrated empathy, compassion, and a commitment to delivering exceptional patient care.
