Job Summary

The Home Infusion Care Specialist is a licensed clinician responsible for coordinating and providing direct infusion care in the home or alternate treatment site. This critical role requires ensuring patient safety while providing continuity of care in compliance with the physician's plan of care.

Responsibilities

  • Patient Care Coordination: Plan, provide, supervise, and document professional nursing care utilizing the nursing process for patients in accordance with physician orders and established policies and procedures.
  • Direct Patient Care: Perform treatments, administer medications, and educate patients and families/caregivers.
  • Patient Assessment and Evaluation: Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical assessment, response to therapy, and any updated physician's orders.
  • Effective Communication: Practice effective and timely communication with all members of the treatment team, including medical providers, patients, and families/caregivers, to ensure continuity of care.
  • Documentation and Reporting: Complete clinical documentation in real-time during nursing visits following nursing standards of practice and applies approved abbreviations and documentation practices as per Naven Health policies.
  • Discharge Planning: Coordinate discharge planning, including preparing discharge summaries with patient instructions and transferring care.
  • Crisis Management: Respond promptly and appropriately to simple patient requests and/or to any adverse event or occurrence during patient visit. Initiate emergency protocols as necessary.
  • Continuous Learning: Attend and/or complete required training modules, in-services, and continuing education to maintain competency and professional licensure.
  • Safety and Compliance: Observe ethical and legal guidelines for safeguarding confidentiality of patient and proprietary Naven Health information, adhering to HIPAA regulations.

Requirements

  • Education and Experience: Graduate of an accredited School of Nursing, current active and unrestricted registered nurse license, and at least 2 years of clinical experience working as a RN in an area requiring training and hands-on experience in infusion therapies.
  • Skills and Qualifications: Demonstrated competency in INS standards, knowledge of vascular access skills, working knowledge of procedures and techniques involved in administering routine and special treatments to patients, and basic knowledge of computer operating systems and software applications.
  • Physical Demands: Ability to lift or move up to 50 pounds, fine motor skills, and visual acuity required.
