Post-Acute Clinical Liaison Specialist

As a Post-Acute Clinical Liaison at WellSpan Health, you will play a vital role in coordinating patient care services in the assigned setting to ensure a seamless continuum of quality care and improved patient outcomes. This position requires a highly organized and detail-oriented individual with excellent communication skills and a passion for delivering exceptional patient care.
Key Responsibilities
• Introduce patients to WellSpan Home Health and Hospice Services, ensuring a smooth transition from hospital to home care.
• Conduct thorough assessments and screenings to determine the most appropriate post-acute setting for patients, maximizing their independence and quality of life.
• Serve as a consistent point of contact for patients and families throughout the diagnosis and treatment process, providing emotional support and education.
• Collaborate with healthcare teams to develop and implement individualized care plans, ensuring timely and effective communication.
• Provide recommendations for additional post-acute support services, including Advanced Illness Management and Remote Patient Monitoring.
• Participate in interdisciplinary nursing rounds and contribute to the development of community and professional education programs.
Requirements
• Associates Degree in Nursing required; Bachelors Degree preferred.
• 3 years of experience in nursing and related fields, with a strong background in post-acute care.
• Licensed Registered Nurse (RN) with a valid driver's license and a clean driving record.
• Basic Life Support (BLS) certification required.
