Role Overview

We are seeking an experienced Senior Clinical Care Coordinator - LPN to join our team in Southeast Georgia. This role is responsible for providing comprehensive care services to individuals with complex needs.

About the Position

The Senior Clinical Care Coordinator - LPN will serve as a primary point of contact for individuals with complex care needs, conducting initial in-person visits and performing comprehensive needs assessments. Key responsibilities include outreach, scheduling, and coordination with ongoing Case Managers to ensure continuity of care.

Key Responsibilities:
  • Conduct initial in-person visits and perform comprehensive needs assessments
  • Provide options counseling and develop personalized care plans
  • Identify potential risks and safety concerns, and coordinate with Case Managers to address immediate concerns
  • Educate participants, caregivers, and family members on available care options and programs
Requirements:
  • Requires an associate degree in nursing from an accredited institution
  • Requires current and valid License Practical Nurse (LPN) license
  • Minimum of three years of nursing experience, preferably in an intake, triage unit or assessment role

Estimated salary range: $55,000 - $75,000 per year, depending on location and experience. The Columbus Organization offers a competitive compensation package, including medical, dental, and vision insurance, as well as opportunities for professional growth and development.
