Job Summary

We are seeking a skilled RN MDS Coordinator to join our team at Ascension. As a key member of our nursing administration team, you will be responsible for collecting data to determine care plans, ensuring the accuracy of patient charts, and working collaboratively with an interdisciplinary team.

Responsibilities
  • Collect data for use in determining care plans, compliance with treatment norms and privacy standards, and in supplying information to third parties.
  • Ensure the accuracy of patient charts and medical data.
  • Work as part of an interdisciplinary team.
  • Maintain current knowledge of federal, state and local standards and regulations for long-term care, such as those for certification, licensure and accreditation, Medicare, and state Medicaid.
  • Participate in all aspects of the CMS survey process including pre-survey preparation, actual survey, and post-survey plan of correction when requested.
  • Promote survey preparedness at all times.
  • Train individuals to develop a comprehensive care plan for facility residents in coordination with the MDS that accurately addresses the needs of residents.
Requirements
  • Licensure / Certification / Registration: Registered Nurse obtained prior to hire date or job transfer date required.
  • Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
Preferred Qualifications
  • Long-term care experience preferred.
