FSRI Community Health Worker

We are seeking a highly motivated and bilingual Community Health Worker to join our team at FSRI. This is a full-time position that offers a $500.00 sign-on bonus within 6 months of employment.
About the RoleThis Community Health Worker position is co-located at Jenks Park Pediatrics office in Central Falls and plays a vital role in promoting equity and inclusion. The CHW will be responsible for actively engaging with residents residing in Pawtucket and Central Falls, identifying areas of need, and ensuring referrals are made to appropriate services. Additionally, the CHW will provide individuals with PPE upon request, COVID-19 prevention materials, and connect them to community-based vaccination events.
Key Responsibilities- Implement Social Determinants of Health and other screening tools to identify areas of need
- Make referrals to appropriate services
- Provide individuals with PPE and COVID-19 prevention materials
- Connect individuals to community-based vaccination events
- Complete home/community visits to support individuals as needed
- Lead Pawtucket Central Falls Community Health Worker Networking Meetings
- Participate in other programmatic meetings
- Associates or Bachelor's degree preferred, High School Diploma or Equivalent required
- Certified Community Health Worker, or ability to be certified within first 12 months of employment
- Bilingual in Spanish required
- Valid driver's license and reliable transportation
- Ability to document in and navigate an Electronic Medical Record (EMR)
- Strong interpersonal and social skills with an ability to collaborate with a variety of individuals
- Ability to provide services virtually, in resident's homes, and community locations
- Ability to have a flexible schedule to include evening shifts and occasional weekends
- Prior experience in behavioral health and/or case management preferred
- Bilingual skills are compensated by an additional 6%, above base pay
- Ability to travel to and from community locations and agency sites
- Ability and willingness to utilize required Personal Protective Equipment as required
- Follow FSRI and Jenks Park Pediatrics professional dress code
- Comply with required immunizations outlined by Jenks Parks Pediatrics staff
- Ability to lift up to 20lbs
- Ability to communicate effectively
