Patient Care Specialist

As a Patient Care Technician at Baystate Health, you will play a vital role in providing high-quality patient care. Your daily activities will be coordinated by the RN, and you will be responsible for performing a variety of technical tasks and bedside functions. Your responsibilities will include direct patient care, hygiene, skin care, mobility, and monitoring. You will also be multi-skilled to meet the needs of the unit in performing duties relative to phlebotomy and EKGs. Additionally, you will be expected to manage equipment and perform or assist in all procedures on the unit.
Key Responsibilities:
1. Provide age-appropriate care to patients. You will demonstrate knowledge of developmental stages for different age groups and apply developmental theories and concepts when implementing care for patients.
2. Monitor basic physiological parameters. You will take and record routine vital signs within the specified timeframe as indicated by periodic review of documentation. You will also obtain and document weights/heights, abdominal girths, head circumference (Pediatric Service) as per physician/nurse order 100% of the time as indicated by quarterly review of 5 records. You will consistently report any significant deviations in vital signs as outlined in the procedure immediately. You will record and report, 100% of the time, any change in patient's normal drainage pattern, i.e. Gastric, wound, blood.
3. Administer treatments and procedures in accordance with BMC policy/procedure. You will change dressings within one hour of specified timeframe in accordance with established procedure. You will provide postmortem care as directed by RN according to BMC policy. You will perform all procedures outlined in nursing procedure manual within area of responsibility in accordance with specifications. You will assist physicians with exams and simple procedures. You will assure identity of patient by checking identification bracelet and identification band prior to performing any procedure.
4. Assist patients with elimination of body wastes. You will make available or offer bedpan, urinal and emesis basin immediately after patient request as indicated by absence of patient complaints. You will accurately label and send specimens of urine, feces and sputum with 30 minutes of specified time frame unless discussed with RN as evidenced by random review of reports. You will accurately measure and record intake and output in accordance with established policy as indicated by random review of intake and output forms. You will administer enemas or Harris Drips as ordered in accordance with established procedure as observed.
5. Perform POC testing
6. Assist in the delivery of ancillary services as directed. You will assist with treatment by following specified treatment plans developed for individual patients. You will monitor patient's condition during treatment and notify appropriate personnel of unexpected occurrences. You will provide information to RN based upon observation made during patient related care. You will provide treatment within the specified time frame. You will document treatments and activities consistently.
7. Communicate effectively with team and patients. You will interact with patients in a respectful and informative manner. You will clearly communicate information related to patient care to the team. You will recognize culturally based patient and family needs.
8. Be knowledgeable of and initiate emergency procedures. You will respond effectively in emergency situations as observed. You will maintain Patient safety at all times. You will communicate effectively with appropriate personnel.
9. Function as a member of the team. You will assist team members with clinical activities. You will provide feedback related to the team process. You will assist team members with non-clinical activities.
10. Competencies and completion and maintenance of clinical recognition program for career ladder level.
