Job Title: Clinical Nurse Manager

We are seeking a highly skilled and experienced Clinical Nurse Manager to join our team at Community of Hope. As a Clinical Nurse Manager, you will be responsible for providing exceptional patient care and leading a team of nursing professionals to deliver high-quality healthcare services.

Key Responsibilities:
  • Develop and implement patient-centered care plans to ensure coordinated care across healthcare providers and settings.
  • Supervise and mentor nursing staff to ensure excellent patient care and team coordination.
  • Collaborate with healthcare providers to ensure seamless care transitions and patient outcomes.
  • Monitor and evaluate patient care quality, identifying areas for improvement and implementing changes as needed.
  • Develop and maintain relationships with community partners to ensure access to comprehensive care services.
Requirements:
  • Bachelor of Science degree in Nursing required.
  • Current, unencumbered DC Registered Nurse license with current CPR certification required.
  • One year of supervisory experience and one year of experience in an ambulatory, primary care, or med/surg setting.
  • Care management and patient education experience preferred.
  • Knowledge of chronic disease processes and health maintenance required.
  • Demonstrated cultural competence in communicating with low-income populations required.
What We Offer:
  • Competitive salary range of $95,000 and up based on experience.
  • Remote work opportunities available.
  • 8-hour workdays with paid lunch.
  • 11.5 paid company holidays, 1 personal floating holiday, 15 days of paid vacation, and 12 days of paid sick leave.
  • Annual performance-based raises up to 5% of annual pay.
  • National Health Service Corps (NHSC) and DC Health Professional Loan Repayment Program (DCHPLRP) participants.
  • Tuition reimbursement, loan repayment for clinicians, licensing reimbursement, and continuing education unit funds for licensed staff.
