Job Summary

As a skilled RN-Home Health, you will provide direct nursing care to patients in their homes, ensuring their comfort, safety, and well-being. This role requires strong communication and interpersonal skills, as you will work closely with patients, families, and healthcare teams to develop and implement personalized care plans.

Key Responsibilities
  • Implement and monitor patient care plans, ensuring timely and effective interventions.
  • Coordinate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Assess and document patient conditions, identifying areas for improvement and developing strategies for enhanced care.
  • Provide education and support to patients and families, empowering them to manage their health and well-being.
  • Collaborate with interdisciplinary teams to develop and implement discharge plans, ensuring a smooth transition to community-based care.
Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of practice.
  • Ability to work independently and as part of a team, with strong communication and interpersonal skills.
  • Strong critical thinking and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work in a fast-paced environment, with a focus on patient-centered care and safety.
