Nurse Specialist

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to provide high-quality care to patients in their homes throughout Northampton County.

We are looking for a compassionate and dedicated RN to join our team as a Home Health Nurse. As a Home Health Nurse, you will be responsible for providing one-on-one care to patients in their homes, focusing on their unique needs and goals.

• Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
• Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for optimal care.
• Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
• Monitor patients' conditions, reporting changes to Clinical or Client Services Manager.
• Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.

• A current license as a Registered Nurse in Pennsylvania.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience is strongly preferred, but not required.
• Graduation from an accredited and approved nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and solid computer skills, with prior experience with electronic medical records (EMR) preferred.
