About the Job:

This is a leadership role within the Southwest Community Health Center, providing comprehensive HIV care coordination services to patients. The ideal candidate will possess 2 years of experience in HIV care and be familiar with Ryan White services.

Key Responsibilities:

  • Manage immunization inventory for clinics and ensure timely re-ordering of supplies.
  • Develop an Anal PAP smear program for all Southwest sites.
  • Assist providers with same-day treatment of STIs and perform three-site testing for Gonorrhea/chlamydia.
  • Work directly with the Principal Investigator on clinic trials.
  • Assist with recapture efforts to re-engage persons lost to care.
  • Train LPNs in internal medicine regarding HIV care.
  • Coordinate specialty services for patients with internal and external providers.
  • Administer medications/vaccines and perform clinical procedures per standing order or clinician written order.
  • Conduct assessments and interventions identified in panel management meetings.
  • Ensure quality assurance, CLIA waived tests, Joint Commission, and Chronic Care model standards are met.

Requirements:

  • A current Connecticut Registered Nurse's license with 2 years of HIV experience and familiarity with Ryan White services.
  • CPR certification (BLS).
  • Strong analytical skills and critical thinking.
  • Excellent interpersonal communication skills and ability to work with a diverse workforce and client base.
  • Proficient in MS Word, Excel, and PowerPoint.
  • Experience with EHR documentation.

Salary Range: $65,000 - $80,000 per year, based on experience and qualifications.
