Key Responsibilities:

  • Provide skilled nursing care to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with interdisciplinary teams to ensure comprehensive care

Requirements:

  • LPN/RN licensure
  • Ability to work flexible hours, including 12-hour shifts
  • Experience working with medically fragile patients
