About the Role:

As a Speech Language Pathologist with At-Heart Home Care & Hospice, you will play a vital role in providing direct care to patients and coordinating with the interdisciplinary team to ensure an optimal care experience. Our Speech Language Pathologists are the heart of our organization, and we are committed to providing exceptional patient care and championing our agency leadership and teams.

Responsibilities:

• Conduct evaluations to assess home environment and identify equipment needs relative to speech/language function
• Identify functional speech defects and establish a plan of care to improve patient's function
• Administer speech therapy program utilizing specialized therapeutic technique and/or equipment and recommend communication devices/aids as indicated
• Maintain accurate and timely documentation
• Participate in case conference as required
• Ensure compliance with all state and federal legal and regulatory requirements

Qualifications:

• Master's degree in Speech Language Pathology with current licensure to practice in state of operation
• 1+ year experience in Speech Language Pathology
• Home health experience preferred
• Commitment to clinical and documentation excellence
