We are seeking a skilled Physical Therapist to join our team at HomeCall, Inc. in providing home health services to our Assisted Living Facilities in the area.
As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, you will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.
You will also be responsible for observing and reporting the patient's response to treatment and any changes in their condition to the supervising nurse and/or physician. Additionally, you will treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
Key Responsibilities:
- Evaluate patient's functional status and physical therapy needs
- Assist physician in development of therapy plan of care
- Observe and report patient's response to treatment and any changes in condition
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
- Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals
Requirements:
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
